age of child, shirt, coat, wader, pant sizes of child, address, phone number,

Willingness to sign release for video production if drawn. |

Willingness to sign release for articles if drawn. |

Willingness to sing insurance waiver if drawn. |

Adult/Child will be responsible for own transportation to Allen-Curtis Wildfowl
Adventures in New Madrid, Missouri. |

No alcohol allowed at any time throughout the weekend. |
| 9. |

The youth, or the adult accompanying the youth, must have taken and passed
their states hunters safety course. |
